Medication review

A second look at what you already take

For people who have been on medicines for years, from more than one prescriber.

Lists grow. One medicine was added for a reason that has since passed, another for a side effect of the first. In a review we read the whole list with you, ask what each is for, and write down what we find.

We do not stop anything in the same visit: any change is discussed first and made one step at a time.

What we look at

Three things in a review

  • Each medicine

What it is for, and whether that is still needed.

  • Doses and timing

When you take what, and whether it can be simpler.

  • Questions to take along

A short list for your pharmacist or other clinicians.

Before you come

What to bring

  • Every bottle or a written list, with doses
  • Over-the-counter medicines, vitamins and supplements
  • The name and phone number of your pharmacy
  • The names of other clinicians who prescribe for you

A review is booked as an evaluation if you are new to us, and as a longer follow-up if you are already a patient.

Tell us how many medicines you take, and we will book the right amount of time.
